Health & Insurance Forms

Please send these REQUIRED swim camp forms to our Camp Administrator, Anne Rueppel, within 2 weeks of registering your child.  The completed paperwork must be received in order for your camper to attend. Please click on the following link to print the forms:

Team Discount Application
 
We offer team discounts ($35 or $50 depending on number of campers) for teams sending 6 or more campers. The Team Discount Application is due by April 15th in order to receive the discount. For more details, and the terms and conditions, click on this link:
 
 
Paper Registration Form
 
If you prefer to register offline with a paper application, please download the paper registration form below. Mail the completed form along with your check or money order for a $250 deposit or the full tuition to Anne Rueppel, our Camp Administrator, at her address listed above. Please note, your camper's spot will not be reserved until your check is received in the mail.
